Please make sure to bring the following documents when you register your child:
- Proof of Residency (PG&E, SMUD, etc.)
- Birth Certificate of Live Birth
- Proof of Physical Examination
- Immunization Records
- Dental Records
- Parent/Guardian’s Full Home Address, Phone Number, Email, City of Residence
- Parent and Child’s Social Security Card
- Income Verification
- Child’s and Parent’s TB Test Results
- Medical Card
- Emergency Contact (Name, Phone Number, City)
Note:
- Waiting lists are maintained when classes are full
- Parent participation is required
